We are committed to making your visits with us as productive as possible. If you are visiting with us for the first time you can fill out your forms before you arrive to help speed up the process. Our forms can be found here. Simply open up the file and print it out and you should be ready to go.
Also, it is always a good idea to confirm with your insurance provider exactly what your coverage options are. A list of providers that we work with can be found here. If you don't see your provider feel free to contact us and we can help you confirm your coverage options.

Jason West graduated from the University of Utah Physical Therapy program in 2002 with a Masters of Physical Therapy. He is also a Certified Strength and Conditioning Specialist (CSCS). He is currently working on a Masters of Business Administration degree from the University of Utah. Jason is well rounded in comprehensive orthopedic physical therapy.
As a professional skier, he brings unique expertise to treatment of skiers of all abilities. He also brings experience to treating other outdoor athletes such as climbers, mountain bikers, runners, and kayakers.
